The Make a Mammogram Promise Official Rules
1. NO PURCHASE NECESSARY TO ENTER, PLAY OR WIN. VOID WHERE PROHIBITED OR RESTRICTED BY LAW.
2. Entry Period: The Promotion begins on September 23, 2011 at 12:00AM Pacific Standard Time ("PST") and ends on October 31, 2011 at 11:59AM (PST) (the “Promotion Period”). The Promotion consists of the Daily Drawing ("The Drawing") and a Spread the Word Contest ("The Contest").
3. Eligibility: The Promotion is open to legal residents of the 50 continental
4. To Enter The Daily Drawing: To make a Mammogram Promise, visit www.mammogrampromise.org and complete all required fields on the online entry form. Entrants must also electronically acknowledge and agree to the terms of the Official Rules and Privacy Policy and submit the online entry form as instructed. All entries submitted in accordance with these Official Rules shall be hereinafter referred to as “Eligible Entries”. Limit one (1) entry per entrant per day. Additional entries can be made by collecting promises in the Spread the Word Promotion (described below).
Registrations, entries, game plays and/or prize claims will be deemed to be made by the authorized account holder of the email address submitted at time of entry and he/she must comply with these Official Rules. The authorized account holder is defined as the natural person who is assigned an email address by an Internet access provider, online service provider or other organization that is responsible for assigning email addresses or the domain associated with the submitted email address. A potential winner may be required to provide the Promoter with satisfactory proof that he/she is the authorized account holder of the email address associated with the potentially winning entry or game play.
5. To Enter the Spread the Word Promotion: Upon making a Mammogram Promise you will be given the option to create a Dashboard that includes the means to invite others to make a Mammogram Promise, including sending a unique URL and setting up a Mammogram Promise widget on a website or blog. Each method works by collecting the email address of those wishing to make a promise and associating it with your Dashboard. You will be credited one promise for each eligible and unique email address representing someone who makes a Mammogram Promise from your email, widget or other method as determined by the Promoter. In the event that you and another contestant(s) "network" the same person, the "networked" person’s promise will count for you and each of the other contestants as long as the "network" person responds completely to each invitation with a valid email address.
6. Prize Winner Selection and Notification: The Daily Drawing takes place every day at 12:01PM (PST) starting on October 1, 2011. To be eligible for that day's drawing, entries must be received by 11:59:59AM (PST). Winners will be randomly selected from all the accumulated entries by the Administrator, a judging organization selected by and contracted with the Promoter whose decisions are final on all matters relating to the random drawing. Entrants need not be present at the time of the drawing to win.
If you receive a winning Daily Drawing email message, you will be directed to follow the directions to complete and submit the online prize redemption form. You will also be sent an email prize confirmation message to the email address provided during registration stating that you may have potentially won the prize indicated in the message and be provided with a link to the prize redemption form. All prize awards are subject to verification of eligibility and compliance with these Official Rules. All prize redemption forms must be completed and returned by November 15, 2011 at 11:59:59PM (PST). Failure to timely claim prize will result in prize forfeiture. Any attempt to use multiple email accounts or other tactics to register or enter more than the stated limits may result in the disqualification by the judges or Promoter from the Promotion and all associated entries and/or prize claims will be void. Entries and registrations generated by script, macro or other automated means or with the intent to subvert the registration or entry process will be void.
7. Privacy: Except as noted in these Official Rules, information collected for this Promotion is used only for the purpose of Promotion administration and winner notification, and will not be re-used, sold or shared in any manner by Promoter or any third parties unless participant has elected to receive additional information and Promotional material from the Promoter or a third party. By participating in the Promotion, participant agrees to all of the terms and conditions of the Official Rules, these Official Rules shall control and govern.
8. The Daily Drawing Prizes: One (1) Daily Drawing prize winner will be selected each day in a computer-generated, random drawing of all Eligible Entries received in the applicable 24-hour period. The Daily Drawing will be conducted by the Administrator using a randomization method selected by the Administrator in its sole discretion. A total of thirty-one (31) Daily Drawing prizes will be awarded during the Promotion Period. If you have been selected as a Daily Drawing prize winner, you will receive a winning email message within 7 days.
The return of any prize email or prize as undelivered or noncompliance with these Official Rules by any potential winner will result in forfeiture of prize. Unclaimed Daily Drawing prizes will not be awarded. Limit one (1) Daily Drawing prize per person. The odds of winning a Daily Drawing prize depend on the number of Eligible Entries received. No substitution, transfer or cash equivalent of the prizes or any portion thereof permitted, except by the Promoter, in which case a prize of equal or greater value may be awarded.

Alderbrook Resort and Spa Trade Scrip: Advanced reservations must be made and are subject to availablity. Parties making reservations must state that they are redeeming trade scrip before requesting rates or availability. Rooms will be charged at regular tack rates. Special offers do not apply. Cannot be combined with any other offer or discount for lodging, food or beverages. Reservation are not automatically guaranteed. Guest must guarantee late arrival with a credit card. Orginal trade scrip must be presented at check-in. No refund or credit from unused portion of the scrip will be given. Scrip may only be used once and will be retained by the reservation agent. This trade scrip will be honored for the purchase of lodging, food and beverages only. Credit card imprint will be taken at check-in. Taxes, tips, telephone calls, spa services and all other goods and services must be paid with cash or credit card. Use of trade scrip cannot be extended beyond the expiration date print onthe scrip. Scrip is not replaceable if lost or stolen. Trade script may be used for a maximum of five rooms per night unless arrangements are made with hotel in advance. Trade scrip will be honored at all times except New Years Eve. Trade scrip expires 12/31/11.

Chocolopolis chocolate tastings: Each certificate is good for a chocolate tasting for up to 6 people. The recipient will simply need to call to schedule the tasting during business hours and we will reserve our counter for their tasting event. We will provide a 15 minute guided tasting of chocolate made by 5 different artisan chocolate-makers who use the finest flavor cacao beans from around the world. Each guest will learn how to properly taste chocolate using all five senses. After the guided tasting, we will provide each guest with a Chocolopolis truffle and a delicious cup of drinking chocolate. Each certificate is valued at $100.

9. General Prize Terms and Restrictions: In no event will more than the number of prizes stated herein be awarded. If due to seeding, technical, typographical, printing, electronic or other errors, the number of valid prize claims received for any prize exceeds the number of such prize set forth herein, Promoter reserves the right to conduct a random drawing to award the stated number of prizes for the prize from among all valid and non-suspect prize claims received for such prize. The value of a prize may be taxable to the winner as income. Winners are responsible for all federal, state and local taxes as well as all costs and expenses associated with prize acceptance and use not specified herein as being awarded. No substitution, cash redemption or transfer of any prize (or portion thereof) is permitted, except by the Promoter who reserves the right, in its sole discretion, to substitute a prize (or portion thereof) of comparable or greater value. Prizes are awarded "as is" with no warranty or guarantee, either express or implied by the Promoter or Sponsors. Any portion of a prize not accepted by winner will be forfeited. If any winner is at least 18 but still considered a minor in his/her jurisdiction of residence, prize may be awarded in the name of his/her parent or legal guardian who will be responsible for fulfilling all requirements imposed on winners set forth herein. All prize details including the color/pattern of all prizes are at Promoter’s sole discretion.
10. Awarding Prizes: The Promoter will award prizes only upon winner confirmation, verification and final approval. Potential winners may be required to provide proof of legal

12. Governing Law: This Promotion is conducted in English (which will be given its everyday ordinary meaning), and any and all disputes will be resolved in accordance with the laws of Washington State without regard for conflict of laws principles. The sole venue for any disputes shall be King County Washington.
13. General Terms: By participating in this Promotion, participants agree to be bound by these Official Rules and the decisions of the Promoter, which shall be final, binding and non-appealable in all respects. The Official Rules will be posted on the Make a Mammogram Promise Website throughout the Promotion Period. Failure to comply with these Official Rules may result in disqualification. The Promoter reserves the right at its sole discretion to disqualify any individual found to be tampering with the participation process or the operation of the Promotion or the Website; or to be acting in any manner deemed by the Promoter to be in violation of the Official Rules; or to be acting in any manner deemed by the Promoter to be unsportsmanlike or disruptive, or with intent to annoy, abuse, threaten or harass any other person and void all associated entries, game plays and/or prize claims. ANY ATTEMPT BY A USER OR PARTICIPANT OR ANY OTHER INDIVIDUAL TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE PROMOTION IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, PROMOTER RESERVES THE RIGHT TO SEEK DAMAGES AND OTHER REMEDIES (INCLUDING ATTORNEYS’ FEES) FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW.
14. Limitations Of Liability: Promoter and the Sponsors assume no responsibility or liability for (a) lost, late, stolen, undelivered, inaccurate, incomplete, delayed, misdirected, damaged or garbled registrations, entries, game plays, prize redemption forms, prize claims, or emails; (b) any incorrect or inaccurate entry information, or for any faulty or failed electronic data transmissions; (c) any unauthorized access to, or theft, destruction or alteration of entries or game plays at any point in the operation of this Promotion; (d) any technical malfunction, failure, error, omission, interruption, deletion, defect, delay in operation or communications line failure, regardless of cause, with regard to any equipment, systems, networks, lines, cable, satellites, servers, computers or providers utilized in any aspect of the operation of the Promotion; (e) inaccessibility or unavailability of the Internet or the Web Site or any combination thereof or for computer hardware or software malfunctions, failures or difficulties, or other errors or difficulties of any kind whether human, mechanical, electronic, computer, network, typographical, printing, seeding or otherwise relating to or in connection with the Promotion, including, without limitation, errors or difficulties which may occur in connection with the administration of the Promotion, the processing of entries, prize claims, prize redemption forms or game plays, the announcement of the prizes, or in any other Promotion-related materials; or (f) any injury or damage to participants or to any other person's computer which may be related to or resulting from any attempt to participate in the Promotion or download of any materials in the Promotion. If, for any reason, the Promotion (or any part thereof) is not capable of running as planned for reasons which may include without limitation, infection by computer virus, tampering, unauthorized intervention, fraud, technical failures, or any other causes which may corrupt or affect the administration, security, fairness, integrity or proper conduct of this Promotion, then the Promoter reserves the right at its sole discretion to cancel, terminate, modify or suspend the Promotion in whole or in part. If terminated, the Promoter may, at its discretion, award the Grand Prize from among all non-suspect, Eligible Entries received for the Daily Drawing up to time of such action.
15. Releases: All participants, as a condition of participation in this Promotion, agree to release and hold harmless Released Parties from and against any and all liability, claims or actions of any kind whatsoever for injuries, damages, or losses to persons or property which may be sustained, in whole or in part, directly or indirectly, in connection with accessing the Web Site, participation in any aspect of the Promotion, or the receipt, ownership or use of any prize awarded.
